To save time during your first visit, feel free to print out these forms and fax or bring them with you. Also, please bring a picture ID and your insurance card. Students must bring a valid student ID in order to get the discounted price.

Patient Forms

We will request you fill our patient forms located online under "New Patients", or you will be provided the forms to fill out upon arriving at our office, so please allow 15 minutes prior your appointment time to complete the required paperwork. The paperwork provides us with general information about yourself and your condition.

History and Consultation

  Here is where we discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for us to learn more about you, your condition and your expectations to determine whether and how we can help you.


  After completing your consultation and determining how we can help you, we will perform a complete examination testing your reflexes, your ability to turn and bend as well as other standard neurologic, orthopedic, postural, and physical examinations. Getting a baseline at this first exam will help us monitor your progress and improvement upon consequent re-exams. Please note however that nothing will be done in our office without your consent.

X-Ray Studies

  Your specific condition may require us to take x-rays to get a better idea of the extend of your condition and help us rule out more serious conditions or assist us in developing the most effective treatment plan for you. We will refer you to a local radiology center, which we work with to get the best professional studies.

Report of Findings

  Once all the information and examinations have been performed, we will give you a report of all findings and explain how we can help you, what we recommend you do at home, at work, at the gym.


  Patients will generally be provided with their first treatment at the conclusion of the appointment. This may include spinal adjustments, physiotherapy, and/or soft tissue therapies. Your first treatment and all treatments thereafter are interactive, where you can express your likes, dislikes, concerns and praises. This is vital for us to tailor your treatment specifically for you and what you will be most comfortable with.
